How They Work

Early hearing aids were analogue in nature but since the 1980’s have largely moved over to digital technology. The general process of how they work however is the same. Starting with sounds in the external environment, the hearing aid first receives the sounds, typically through some form of microphone. Received sound waves are then recorded and converted. In older analogue devices the conversion process resulted in electrical signals or impulses. With digital aids the recorded sounds are converted into numerical codes, which the hearing aid’s amplifier then analyses. Hearing aids are pre-programmed by the manufacturer based on the severity and nature of the hearing aid user’s hearing loss. Finally, the now analysed and amplified information is converted back to sound waves and sent to the brain via a speaker. Where these three parts of a hearing aid – the microphone, the amplifier and the receiver – are placed depends on the style of the aid.

Styles

Although all hearing aids utilise the same basic components to collect sounds from your external environment and pass them along to your brain, there are different styles available. The differences across styles centre on where the hearing aids are located and their size. Ranging from smallest to largest here are some currently popular styles:

Hearing Aid Styles: Advantages And Disadvantages

Hearing aids placed within the ear canal are the least visible but their size leads to some limitations. Completely-in-canal aids – those inserted the deepest into the canal – have shorter battery life and no room for volume adjustment controls. In-canal aids do not go as deep into the canal so they can include volume control and other features. However, due to their placement and small size, these features can be difficult to adjust. Half-shell aids fit into the lower half of the ear bowl and their larger size makes them easier to adjust. Full-shell aids fill the entire ear bowl and are the most visible of the in-ear aids. However, their size enables larger and longer-lasting batteries and additional and easier to adjust features, such as directional microphones. Behind-the-ear aids place a larger amplifier directly behind the ear, attached to the ear with a hook. Sounds are received and analysed here and then sent to an ear mould placed within the ear canal. This type provides the highest amplification available in hearing aids but they are also very visible. Open Fit is another behind-the-ear style that is smaller, less visible, and does not place an ear mould into the canal. As you would expect, decreased size leads to decreased functionality, smaller batteries and fewer and smaller controls.

Should You Get One?

If you think you might have a hearing loss, go and see your local doctor. Many people are amazed when their hearing loss disappears after a blob of earwax is removed! If no obvious medical conditions – such as infections or visible tumours – are present, your doctor should then send you to a specialist. Otolaryngologists focus on eye, ear, nose and throat disorders and can more accurately diagnose the specific causes of the hearing loss. Audiologists will test your hearing to determine the nature and severity of the loss. These test measurements will later be used to program the hearing aid you select. Audiologists are generally more knowledgeable when it comes to answering questions about which type of hearing aid would be the best fit for your particular level of hearing loss. You should be aware that no hearing aid will return you to normal hearing. With a hearing aid you will receive more and louder “sounds” – including distracting background noise – but the sounds will come across to your brain as a little strange and different. To understand hearing loss, you must first understand how we hear. The mechanisms that cause hearing are somewhat uncomplicated. Hearing takes place when sound waves travel through the structures inside the ear and are converted to nerve signals that can be recognised by the brain. There are three major areas that make up the ear – the outer ear, middle ear, and inner ear. Sound waves first pass through the outer ear and cause the eardrum to vibrate. The eardrum and the middle ear (which also contains three small bones known as the hammer, the anvil, and the stirrup) cause an amplification of the vibrations from the eardrum on their way to the inner ear. Once the vibrations reach the inner ear, they pass through fluid found in the cochlea, which is a small structure shaped much like a snail inside the inner ear. The cochlea are attached to nerve cells which, in turn, are covered with thousands of miniscule hairs that assist in the translation of the sound vibrations, turning them into electrical signals that are relayed to the brain. Different sounds are transmitted in different ways, allowing the brain to interpret a variety of messages through the ear.

Hearing Loss: Causes

While the buildup of earwax and earwax blockage can cause hearing loss in people of any age, most hearing loss is the result of damage to the delicate cochlea. The cochlea – or more specifically the tiny hairs in the cochlea – become bent or broken over time which can lead to degeneration of the nerve cells attached to the cochlea. • Ages 18 to 24 months – The child’s vocabulary should be at fifty words or more during these months and should easily respond to “yes” and “no” questions.
• Ages 2 to 3 years – The child’s vocabulary should be on average 270 words. The child at this age should be able to sing terse songs and even make up three to four word sentences.

An ounce of prevention is worth a pound of cure. If a parent or guardian suspects that their child is unable to perform the above mentioned tasks, set an immediate appointment with the child’s paediatrician.

Signs And Causes Of Hearing Loss In Children

There are many causes and signs of hearing loss in children, which is all the more reason to have a child’s hearing tested as soon and as often as needed. A few of the signs and causes for a hearing test include:

• Severe jaundice after the child has been born
• Maternal sickness during the pregnancy (e.g. rubella)

The Hearing Process

Hearing begins when our outer ears pick up sound waves from the external environment and sends them to the middle ear. Here the ear drum begins to vibrate and – working together with three tiny bones – transmits the vibrations to the inner ear where they are converted to nerve impulses that end up in the brain. The brain recognises these impulses as sound. The common terms for the three bones of the middle ear -known collectively as the “ossicles” – are hammer, anvil, and stirrup. The correct medical terms are malleus, incus, and stapes. The inner ear holds the cochlea which is filled with fluid and hair cells. The vibrations received from the middle ear pass through the cochlear fluid and cause the hair cells to begin moving. It is this stimulation and movement that leads to the creation of the electrical nerve impulses that go to the brain. Different external sounds lead to different vibrations which stimulate different sections of the cochlea which results in the transmission of impulses that allow the brain to distinguish among a variety of sounds.

Otosclerosis And Hearing Loss

The abnormal growths or lesions resulting from otosclerosis most often affect the last bone in the middle ear – the stirrup or stapes. The growths affect the bone’s ability to vibrate, progressing to the point where the stapes gets stuck and won’t vibrate at all. This can have a serious impact on sound waves passing through to the inner ear. The change in both the number and the quality of the sound waves transmitted leads to the hearing loss. In rare cases, otosclerosis can impact the nerve fibres and cells within the inner ear.

Causes Of Otosclerosis

Researchers cannot point to a single cause with 100% accuracy but it appears genetics plays a major factor. Otosclerosis runs in families so if both of your parents have or had it you have about a 50% chance of developing it as well. Research also indicates a possible link between otosclerosis and hormonal changes occurring during pregnancy. Viral infections – like measles, influenza, and sinusitis – may also have links to otosclerosis.

Symptoms Of Otosclerosis

Hearing loss and tinnitus are the most common symptoms. Remember that the hearing loss can be gradual and take a long time before it becomes dysfunctional enough to be noticed. Tinnitus is most frequently thought of as “ringing in the ear”, but the sensations experienced may also include hissing, buzzing, or even roaring sounds.

Diagnosing and Treating Otosclerosis

Your doctor will typically refer a patient suffering from hearing loss to an otolaryngologist – an ear, nose, and throat specialist.  These specialists can determine if there are other causes of the hearing loss that could produce similar symptoms. Audiologists are usually involved in the diagnostic process as well. They have a variety of tests and procedures to evaluate the functioning of the middle and inner ear.

Once otosclerosis has been diagnosed there are two treatment options – surgery or hearing aids. The surgical procedure – called a stapedectomy – involves removing the stapes and replacing it with an artificial bone, frequently made of Teflon. In some cases the stapes is left in place and simply bypassed in favour of the implanted prosthetic or artificial device.

Hearing aids can be appropriate where the hearing loss resulting from ostosclerosis is less severe. A hearing aid acts as a sound amplifier. The audiologist who performed the diagnostic tests should be able to outline the types of hearing aids available and which would be most suitable to the needs of the person with the condition.

Research On Otosclerosis

Much of the current research emphasis is on identifying the genetic structure that might carry the disease from parent to child. In addition, laser surgery techniques applicable to stapedectomies are being improved. Researchers are also working on improving the artificial bones used in the process.

This test should not be used as a diagnostic tool, nor should the patient attempt to derive conclusions from the data that is provided as a result of this test. Your hearing professional will be able to examine the test and answer any questions that you may have in regards to this issue, but you just cannot use this test as a diagnostic tool.

Different Types of Tympanograms

There are three different types of results to be garnered from tympanometry. Type A Tympanograms are considered to be very good and means that things are functioning very well. There is proper and regular pressure in the ear, with normal movement of the ear drum and conductive bones. This is what you want to hear because it means that the ear is functioning properly. Type B and Type C indicate that there may be problems and issues in the middle ear. Those potential issues could be anything from scarring of the tympanic membrane to lack of contact between bones, perforation of the tympanic membrane, fluid build up, or at the dramatic end of the spectrum, a possible tumour in the ear. Any of these issues will need further testing and possible medical treatment to ensure no long term damage to your hearing.

How Is The Test Administered?

The sounds are played over a headset that is placed directly on the test subject’s ears and fitted snugly so that they rest gently on the ears. This method of measurement and delivery is commonly referred to as “air conduction”. The sounds travel from the headphones, through the air, into your ear canal, through the middle ear and finally arriving at the very sensitive inner ear (more commonly referred to as the cochlea). The sensitivity of the cochlea itself can be tested by a few different methods. One of those methods involves placing a small vibrating device on mastoid bone behind the ear itself and again measuring the softest sounds that the test subject can pick up. The difference in this method as compared to the previous is the middle ear is skipped in the process and is referred to as “bone conduction”. Once these tests have been administered, the results can be transferred onto an audiogram, which will assist you in finding out where the location of your hearing problem is. This allows you to potentially remedy the issue, or at least take steps to assist in restoring it.

What Is An Audiogram And What Purpose Does It Serve?

An audiogram is a way of representing a person’s hearing loss on a graph. The numbers at the very top of the audiogram graph range from 125 to 8000. These numbers represent the frequencies or different pitches in sound that are measured. Frequency is measured on the audiogram along the horizontal axis in Hertz (Hz) while loudness is measured in Decibels (Db) along the vertical axis. The range tested is usually between 250 and 8000 Hertz as this represents and contains the most speech occurrence. Air conduction levels are also marked on the chart, differing between the two ears with an X marking one ear, and an O marking the other ear. Bone conduction levels are also marked on the chart. The audiogram is essentially a “snapshot” of your hearing. It will allow the medical professional administering the exam to determine strengths and your weaknesses and if monitored regularly you will be able to stay ahead of the curve in order to notice changes in your hearing levels. Hearing tests that use audiograms (such as pure tone audiometry) will allow you to be proactive in working with a hearing professional on finding solutions to any problems that may occur.

What Are They?

A cochlear implant is a small electronic instrument that enables both adults and children with significant hearing loss to pick up certain sounds. Although they can help the deaf to make out sounds like music, environmental noise and the human voice, they do not bring back normal hearing. There are two main parts to the implant. An external piece fits directly behind the ear and an internal piece is surgically implanted under the skin.

How Do They Work?

The most familiar electronic device available for the hard of hearing is the hearing aid. Whereas a hearing aid amplifies the sounds it picks up, cochlear implants operate on a completely different principle. They use the sound signals received to stimulate the auditory nerve itself. A microphone, which is placed behind the ear picks up external sounds and passes them along to a speech processor, which is also located behind the ear.

The processor translates the sounds which are then transmitted to the surgically implanted parts within the ear. This surgical process involves cutting a flap of skin behind the ear where a receiver/transmitter is placed and inserting a special electrode into the cochlea. The receiver then receives signals from the speech processor, converts the information into electronic impulses and transmits them to the electrodes placed in the inner ear.

These electrodes then provide the stimulus to the auditory nerve. Finally, the auditory nerve signals the brain in such a way that the signals are recognised as sound. However, users of cochlear implants must learn to interpret those sounds. Therapy typically follows cochlear implant surgery to help with the relearning process.

Who Can Use Them?

Not all people who are hearing impaired or suffering from hearing loss can benefit from cochlear implants. Those who for a variety of reasons have experienced deterioration of auditory nerve function would get no benefit from them. Remember, cochlear implants work by stimulating the auditory nerve itself.

Most experts agree that these implants are suitable for the severely hard of hearing – those who can detect sounds between 70 and 90 decibels – and for the profoundly deaf – those who can detect sounds of 90 decibels or above. Those who lost their hearing as adults can learn how to associate the brain signals they receive with sounds they recall from earlier in life. Young children generally need extensive therapy after implant surgery to achieve maximum benefit.

How to Get Them

There are Cochlear Implant Centres located around the country, many associated with the medical centres of large universities. The post operative phase of a cochlear implant can be much longer than other surgeries, requiring extensive therapy with speech pathologists, audiologists and so on. Some adults may be unwilling or unable to extend the effort required for success. Cochlear implants in children will require a great commitment of time and effort from parents and other family members. This is not a procedure to be taken lightly.

Future Developments

As with many electronic devices, the future improvements in implant technology involve increased miniaturisation. As parts become smaller, they can be made more powerful. For example, as the size of the electrodes implanted in the cochlear implant decreases, more can be used. Some predict that in the near future up to ten times more electrodes than we use today will be able to be implanted.
